Student Accident Plan Information

Please see the St. Anthony’s School Nurse and complete the claim form within 24-72 hours after an accident or injury.  Claim forms can be found in the nurse’s office.

Note: Treatment for injuries should occur within 30 days. Parents must follow the guidelines established by their primary insurance carrier. Claims must be submitted within 180 days after the date of the first treatment.
